Is Stored Blood Bad Blood?

It is becoming increasingly clear among blood bankers and others involved in transfusion medicine that red blood cells (RBCs) don’t store well—but what specifically happens during storage?
  • Depletion of 2,3-DPG during refrigeration, causing RBCs to retain oxygen more avidly and potentially scavenge oxygen from tissues
  • Instability of SNO hemoglobin, potentially creating a situation in which RBCs are unable to assist in microvascular regulatory flow
  • Alteration of RBCs, in which they become sticky, rigid, and undergo a change in shape, thereby affecting hemodynamics and microcirculatory function
  • Increase in red cell membrane phosphatidylserine, which potentially makes cells prothrombotic
